Physical Information
All students who plan to participate in extracurricular sports for the 2022-23 school year must have a new physical exam (completed AFTER April 1st, 2022) from a licensed physician. All forms (available below) MUST BE COMPLETED in order to be eligible to attend tryouts or practice.
Forms Needed:
– 2022-23 IHSAA History Form & Physical Exam (Total of 3 pages)
– RankOne Online Lynhurst Signature Form
